Coming in for yourself
Reach out and we'll set up a time to sit down. Bring your Health First Colorado card and a photo ID if you have them. If you don't, come anyway. Sorting that out is something we help with.
Referring someone you work with
Send us your contact info through the form, or call or text, and we'll follow up within one business day to get the details. Once the person comes in, they can sign a release so we can keep you updated.
Worried about someone close to you?
We can't sign someone up on their behalf, but we're glad to talk through how to bring it up. Passing along our number is a good place to start.
